  2. JAMB result/acknowledgment slip
  3. A’ level results, diploma, or IJMB (as applicable)

  7. Certificate of birth/declaration of age

All these documents should be neatly packed in a quarto-size envelope, clearly labeled with the candidate’s name, department, and JAMB registration number.
