- Uploading O’Level Results: To ensure that your admission process is smoothly processed, it is mandatory to upload your O’Level results on JAMB CAPS (Central Admission Processing System). Failure to do so may result in your admission not being processed. So, make sure you upload your O’Level results before the stipulated deadline.
- Screening Fee: Candidates are required to bring a screening fee of two thousand naira (N2,000.00) to the venue. This fee covers the administrative costs associated with the screening exercise.
- Evidence of Change to FUHSA: Candidates are also expected to present evidence of changing their course/institution to Federal University of Health Sciences, Azare (FUHSA) as their first choice. This is an essential requirement for eligibility in the screening exercise.
- Academic Credentials: Remember to bring both the original copies and photocopies of your academic credentials for the screening exercise. These credentials may include your WAEC, NECO, or any other relevant examination results.
- Passport Photograph: Candidates are requested to bring along one (1) passport photograph. This photograph will be required for administrative purposes during the screening exercise.
